Local No.4 Check-Off

 

It is very important that checkoffs be filled out correctly at your place of employment, and given to the Shop Steward, who will then remit it to the office.

The information on the checkoff is used by the  Fund Administration to upload information to your Annuity Account at Mass Mutual.

Information such as your Social Security Number, your address, and your date of  birth is crucial to this process.
